Winona Community HUB has been granted Level 1 Certification with Pathways Community HUB Institute (PCHI). PCHI was founded in 2015 as the nation’s only certifying body for community-based care coordination Pathways Community HUBs. PCHI is a nonprofit certification organization that serves a broad range of community care coordination service networks.
Winona Community HUB’s certification will remain in effect from September 13, 2022 through July 9, 2024. According to PCHI, by pursuing and achieving Level 1 certification, Winona Community HUB has demonstrated that it meets the majority (at least 90 percent) of the national Standards for quality community care coordination services and is committed to pursuing excellence. Further, a Pathways Community HUB receiving a two-year term of certification has put itself through a rigorous peer review process and has demonstrated to PCHI Model Certification reviewers its commitment to offering programs and services that are measurable, accountable, and of the highest quality.
Winona Community HUB impacts drivers of health in Winona County with a comprehensive strategy utilizing the PCHI model, data-measuring software, and community health worker positions within community-based organizations to reduce barriers to services. The Winona Community HUB helps participants identify needs, confirms needs they would like to address, and connects them with resources and services to address those needs.
Winona Community HUB is managed by Live Well Winona and is contracted within the Winona Health Foundation. Community based organizations that support community health worker positions within the HUB network include:
